      Stating: "Occupational Therapy evaluation & treatment"
      Include child's date of birth, diagnosis, & diagnosis code
      Must be renewed every 6 months

3. Download, complete, & return registration forms:
       Registration letter
       Notice of privacy
       Registration form
       Intake questionnaire

4. We will contact you to schedule an appointment
for your child's initial evaluation
